Injection site rotation planner

Lay out an injection site for every weekly dose over 8, 12 or 16 weeks from a start date, cycling the labelled regions (abdomen, thigh, upper arm), alternating sides, and moving the spot within each zone so no spot repeats until the zone has been used four times. Two rotation patterns, a printable table with a tick column, and the label rules underneath. No dose logic at all.

Your plan

Every later week falls on the same weekday. Dose is not part of this tool.

The labels name three regions: abdomen, thigh, upper arm. Untick anything you cannot reach or have been told to avoid.

Where to inject, week by week

Enter the date of your first injection.

Formulashown in full, nothing hidden
week n date = start date + 7 x (n - 1) days
zone for week n = ordered zones[(n - 1) mod number of zones]
spot for a visit = spots[(visits to that zone so far) mod 4]
spots = upper outer, lower outer, upper inner, lower inner
rest per zone = number of zones selected (weeks between uses)

Ordering: "alternate sides, then move region" lists abdomen left, abdomen right, thigh left, thigh right, arm left, arm right (only the zones you ticked). "Change region every week" lists all left zones by region, then all right zones. Either way consecutive weeks are never the same zone.

There is no dose arithmetic in this tool. It plans location only.

Assumptions, checked September 4, 2026

  • Injection regions are the three named on the Zepbound, Mounjaro, Wegovy and Ozempic labels: abdomen, thigh, upper arm.
  • The labels say to rotate injection sites with each dose and not to use the same spot each time; they do not prescribe a specific rotation order. The orders here are two reasonable ways to satisfy that instruction.
  • Four spots per zone and a minimum 1 cm gap between injections follow the FIT recommendations (Frid 2016), written for insulin and applied here as general subcutaneous injection practice.
  • Upper arm: the labels say another person should give the injection in the back of the upper arm.
  • One injection per week is assumed, as labelled for all four products. If your prescriber has set a different interval, the dates will not match.

This calculator is an arithmetic aid for medication a licensed prescriber has already prescribed. It does not recommend a dose. Confirm any result with your prescriber or pharmacist before use.
